I.
Port entry formality
When carrying out
the necessary formalities for entry into ports, captains of ships shall
have in hand the following papers:
a/ Papers which
shall be produced upon request:
- Certificate of
registration or certificate of nationality of the ship
- Certificate of
international load lines, certificate of ship capacity, and other certificate
relating to seaworthiness and preventive measures in respect of environmental
pollution which have been issued by a competent ship registration agency
- License to operate
the radio of the ship
- International
certificates of vaccination of crew members and passengers
- Certificate of
extermination of rats or a certificate stating the reasons for exemption
- List of goods
in transit or temporarily imported for subsequent export, certificates
exempting such goods from quarantine formalities
- Passports or
list of crew members, and passport of passengers
- Certificates
of qualification of the captain, the chief engineer, the chief radio
officer, the chief lifeboat pilot, and all other officers
- Insurance certificate
in respect of civil liabilities of the ship owner in cases of the ship
engaged in transporting oil, petroleum products, or other dangerous
cargo.
- Permit issued
by the Prime Minister of the Government of Vietnam permitting entry
into Vietnam in cases where the ship is nuclear-powered
- In cases where
the ship engaged in fishing vessel, a fishing license issued by the
Aquatic Ministry of the Government of Vietnam permitting fishing activities
in Vietnamese territorial waters
- Declaration of
foreign currency and list of luggage of crew members in cases of foreign
ship, or a Vietnamese ship returning from a voyage abroad.
b/ Papers and
documents which shall be submitted:
- Exit permit of
last port one copy
- Quarantine certificates
one copy
- Certificates exempting
disinfect of cargo one copy
- Arrival declaration
form two copies (standard form)
- Declaration of
health two copies (standard form)
- Declaration list
of personal belongings one copy (standard form)
- Layout of cargo
and inventory of cargo five copies (standard form)
- Declaration of
dangerous cargo five copies (standard form)
- Declaration of
equipment and instruments used during the period at port five copies
(standard form)
- Declaration of
storage reserves five copies (standard form)
- Quarantine declaration
in respect of plants and livestock one copy (standard form)
- List of crew
members five copies (standard form)
- List of passengers
or other persons traveling on board five copies (standard form)
- Declaration of
foodstuff and food reserves one copy (standard form)
- Applications to
go ashore of crew members and passengers one copy (standard form)
II.
Port exit formality
When carrying out
the formalities to exit ports, captains of ships shall have in hand the
following papers and documents:
a/ Papers which
shall be produced upon request:
- Receipts or other
documentary evidence of payment of all related debts and proof of seaworthiness.
- Passports or
list of crew members, and passports of passengers
- Certificates
of qualification of the captain, the chief engineer, the chief radio
officer, the chief lifeboat pilot, and other officers.
b/ Papers and
documents which shall be submitted:
- All permits issued
by the Port Authority or other State administrative organizations or
agencies at the port to the ship, its crew members or its passengers
during the ship's stay at the port (for the purpose of withdrawal of
these permits)
- Departure declaration
form two copies (standard form)
- Packing list
of cargo on board three copies (standard form)
- Declaration list
of personal belongings one copy (standard form)
- Declaration of
health one copy (standard form)
- List of crew
members and passengers five copies (standard form)
No later than six hours prior to the departure of the ship, the captain
of the ship shall submit to the Port Authority the departure declaration
form
